Item upgrades are a way to boost the stats of an item. Based on the type of upgrade it could increase an item's Mark Level or Rarity, and also boost other stats.

Item Level

Using the item level can help players compare the strength of equipment when they decide to buy or sell. It's an excellent gauge of an item's worth and the amount of damage it will be capable of causing. The level of an item can be used to evaluate items when looking at a vendor's inventory.
The item level can be viewed by pressing the ALT key on your PC or using the left stick (L3) on XB1 and PS4. This will show the item's full description, including the ilvl. You can also locate it by hovering over an item and typing /itemlevel into chat.
There are a few theories about how the item level calculation is done. One theory is that the item level is a number derived from a formula that calculates the amount an item's stats are worth. It takes into consideration that different advantages are valued differently and that gems in sockets increase the value of the item. This could result in an item's ilvl calculated, which is typically very close to Blizzard's actual item level.

Item Stats
The stats of an item can be the difference between a good or bad item for players. In addition to Rerolling affixes and rerolls, players can select which stats to be kept on an item when it is upgraded. The possible ranges of stats for an item are determined by the level of the item, however every item is guaranteed to have at least one stat within that range, unless it's an exclusive item or the result of a God roll from a boss in a raid.

Item Luck

Furthermore some items, like the Hollow weapon, which is infused with Hollow can be outfitted to increase the damage dealt by any damage-dealing skill by five percent. These items can be purchased in the shops or made by using the appropriate ingredients.
Item Failure
Item upgrades are expensive and not all upgrades will succeed. However, there are cash shop items that allow players to boost their chances of success or even prevent them from losing an item if the upgrade fails.
When you upgrade an item, the success rate is displayed in the Honing UI. The percentage is the probability of successfully upgrading the item. This can be increased by using Additional Materials or Special materials. The probability of success can only be increased by so much, and the chance that an upgrade will fail can't exceed 100%..
The Luck of an item can also influence the success of an upgrade. Every item has an initial success rate of 25% and each item with Luck increases the chance of success by five percent.
Note that item upgrades are rendered above the inventory items which means that enchantment glints and other effects can be obstructed by the overlays. If you wish to disable the upgrade overlays, the config option decoration.renderUpgradeOverlays can be set to False in the Item Upgrader CORE client config.
